  • Advance Care Planning & Health Care Consent for Primary Care

    Welcome to the course on Advance Care Planning & Health Care Consent for Primary Care.The course is intended to increase your knowledge on the process of advance care planning and the legislation that shapes it through the delivery of core curriculum, interactive activities and case studies.  This course requires registration and enrollment and is restricted to certain health care professionals.

    There is no narration with this module and will take approximately 45 minutes to complete.

    Lesson 3 identifies possible gaps and barriers which impact the ACP process, and identifies differences between goals of care, advance care planning and treatment plans. Tips and strategies are provided to facilitate ongoing conversations.

    A discussion forum question will be posted at the end of the course. Participants are invited to post their responses, share ideas and experiences.
